These Terms of Service (“Terms”) govern your access to and use of https://www.phoenixbhw.com (the “Site”), operated by Phoenix Behavioral Health & Wellness (“Phoenix”). By using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

Not medical advice

The Site is provided for general information about Phoenix and the services we offer. It does not provide medical or mental health advice, diagnosis, or treatment, and it is not a substitute for consultation with a qualified professional. Reading this Site or corresponding with us does not create a provider and patient relationship. That relationship begins only when Phoenix and you have completed an intake and agreed in writing to provide and receive care.

Emergencies

Phoenix is an outpatient provider and does not offer emergency, crisis, or after-hours services. The Site is not monitored around the clock. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department. For free, confidential crisis support at any hour, call or text 988.

Communications through the Site

Email sent to us is not encrypted end to end and should not be used to transmit protected health information or other sensitive details. This Site does not collect messages itself, so anything you send reaches us through your own phone or email provider. We aim to respond to inquiries received during office hours the same business day, and to after-hours messages within one to two business days, but we do not guarantee a response time.

Calling or emailing us does not by itself create a provider and patient relationship, and it does not mean we have accepted you for care. A relationship begins only when we have completed an intake with you and agreed to provide services. Please do not rely on an unanswered message as though it has been received and acted upon. If your situation is urgent, call us at the number on this Site, and in an emergency call 911 or dial 988.

Consulting and advisory services

Information on the Site about organizational, leadership, and public sector advisory services is descriptive only. It is not an offer, and it does not constitute legal, compliance, accreditation, or financial advice. Advisory engagements are governed exclusively by a separately executed written agreement.

Acceptable use

• Do not use the Site for any unlawful purpose or in violation of these Terms.
• Do not attempt to gain unauthorized access to the Site, its servers, or any connected system.
• Do not interfere with the operation of the Site, including by transmitting malicious code or automated abuse.
• Do not submit content that is unlawful, harassing, defamatory, or that infringes the rights of others.

These Terms are governed by the laws of the State of Texas, without regard to its conflict of law rules. Any dispute arising from these Terms or the Site will be brought in the state or federal courts located in Grayson County, Texas, and you consent to that jurisdiction.
